Scheduling:
Due to the nature of our studies, all examinations must be scheduled through the referring office. Please have the following information available when scheduling an exam:
Written orders are required for all scans requested.
See important Referral Reminder for more information.
- Patient's full name
- Social Security Number
- Telephone number(s) - both day & evening, including area code or complete address
- Date of Birth
- Surgical History
- Exact examination requested (please specify studies using a contrast agent)
- Type of Insurance (Verifying this information when scheduling can save your patient unnecessary delays in pre-approvals)
- Any previous x-rays, mylograms, ultrasounds, CT or MRI studies
We understand that we are requesting a good deal of information, but this is for the safety and convenience of your patient.
